My vaccination form asks for dates I can't find. Should I leave them blank?

Health category photographHealth

Mum looked at the new-job form and said, "You had all your childhood vaccines. Put yes."

The form for my US childcare job asks for dates and supporting records. Mum remembers taking me to appointments, but the folder she found contains school photos and a receipt, not a vaccination record. My old clinic has closed.

The employer's health office wants the paperwork next week. I do not want a blank section mistaken for refusing vaccination, but I also do not want to turn a family memory into dates I cannot support.

[CDC suggests checking previous providers, saved documents and state immunization records, then discussing next steps with a clinician if records cannot be found](https://www.cdc.gov/vaccines-adults/recommended-vaccines/keeping-vaccine-records-up-to-date.html).

What should I tell the health office while I search? I need them to distinguish "I don't have the record yet" from "I know I never received it," and I need Mum to understand why I am not filling the gaps from memory.

Ask the health office how it wants unknown dates recorded and what evidence it accepts. "My family recalls childhood vaccination, but I am still locating records" describes what you know without making up a date or treating the recollection as official documentation.

Mum can be a useful partner in the search even if her memory cannot fill the boxes. Ask whether she remembers the provider's name, a school that held a copy or a vaccination card in another folder. That gives her something more constructive than defending "I took you."
